INFORMATION FOR PATIENTS COMING TO OUR OFFICE:
Office space will be limited. We ask unless assistance is needed or you are accompanying a minor that you are alone.
Upon office entrance, we are requiring a mask or similar effective alternative be used. Non-contact temperatures will be taken, also a screening questionnaire before your appointment can continue. If you have a temperature of 100 degrees F, or answers regarding your screening are not within our protocols, you will be asked to reschedule. If you are symptomatic prior to your appointment, call us and we will reschedule.
Patients canceled/not seen due to our closure WE WILL BE CONTACTING YOU TO SCHEDULE A NEW DATE.
As stated above, we are limiting the number of patients to be seen and allowed in office at one time. If you need to have an adjustment, replacement or pick up, we ask you call ahead, we are NOT allowing walk-ins.
PLEASE HAVE ALL CURRENT DOCUMENTATION (Insurance cards, ID’s, etc) ready and available when you arrive. We are asking you come 15 minutes early.
Our actions for safety include but are not limited to: seeing one patient per hour, with our last appointment at 4:00 p.m. to ensure proper sanitizing between patients. We will be disinfecting thoroughly daily, while common and high contact areas will be disinfected in-between patients, including examination instruments, flooring and all surfaces. Staff will also be wearing protective gear and maintaining distance.
This will be a slow thorough process until the stay at home order is lifted. We appreciate your cooperation and understanding on this matter as you, our patients, and staff is of upmost importance to us! We are all in this together!
